A staphyloma is an abnormal protrusion of the uveal tissue through a weak point in the eyeball. The protrusion is generally black in colour, due to the inner layers of the eye. It occurs due to weakening of outer layer of eye cornea or sclera by an inflammatory or degenerative condition. It may be of 5 types, depending on the location on the eyeball bulbus oculi. In the anterior segment of the eye, involving the cornea and the nearby sclera.

Staphyloma is the term for a thinning of the outer, white coat of the eye the sclera in which the underlying pigmented tissue then adds its color to the thinned sclera, giving an appearance of bluish to almost black color. Staphyloma occur in the front of the eye mostly as a response to trauma or infection in which the scleral architecture has been disturbed and the internal pressure of the eye stretches the weak point causing the protrusion and typical appearance.
Rarely staphylomas can be caused by surgical weakening of the sclera at some point. Posterior staphylomas are more commonly congenital at birth or as a manifestation of more extreme myopia nearsightedness. The congenital staphylomas occur near the optic disc which is commonly of normal size, but frequently the vision is substantially lowered in these cases.
When related to myopia, the vision correction or disturbance is usually from the myopia alone although the staphylomatous protrusion in the back of the eye can contribute to visual distortion. There is no specific treatment for this condition but some have applied banked sclera to reinforce anterior staphylomas that appear threatening to erode into the eye. Ask a New Question. We describe an unusual case of acquired anterior staphyloma in a patient addicted to crack cocaine. At the beginning of his crack cocaine abuse, he noticed redness and irritation of his eyes. Over the next 4 months, the patient also noticed the onset of decreasing visual acuity in his right eye OD. Initially, his visual acuity was light perception in OD, and slit-lamp examination revealed a corneal infiltrate with a peripheral perforation and an iris prolapse. The patient was hospitalized to ensure compliance with the prescribed treatment and was advised to undergo therapeutic keratoplasty; however, the patient left the hospital against medical advice and was lost to follow-up for the next 6 months. He returned with complaints of photophobia and the inability to close his right eyelids.
